Unite Iowa is a community data exchange of health and social care providers, sponsored by CyncHealth. Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. The United Way of Muscatine is a convener for the network. The Iowa Health Information Network is an implementation partner.
Organizations offering billable services will enter into a participation agreement to data-share with the health information exchange platform before joining the network. There may be a cost associated with data-sharing, but no additional charges to join Unite Iowa.
The platform is free for community-based organizations.
Unite Iowa is currently serving Adams, Harrison, Mills, Montgomery, Pottawattamie, Crawford, Ida, Monona, Muscatine, Shelby, Woodbury, Lyon, Plymouth, and Sioux counties.
Unite Iowa is part of a groundbreaking multi-state initiative, including Unite Kansas, Unite Minnesota, Unite Missouri, Unite Nebraska, Unite North Dakota, and Unite South Dakota, sponsored by CyncHealth. This work ensures that communities and individuals can seamlessly access much-needed resources and services, such as food, employment, transportation, and housing, through collaboration with local community-based organizations. Partners are connected through a unified network, powered by Unite Us, to support their clients across state lines.

The COVID-19 pandemic made it clear to our community just how valuable a shared client system would be.
