About Us

Unite Iowa is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Iowa is sponsored by CyncHealth. The network is supported by a Iowa-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Organizations offering billable services will enter into a participation agreement to data-share with the health information exchange platform before joining the network. There may be a cost associated with data-sharing, but no additional charges to join Unite Iowa.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Unite Iowa is now live statewide and continues to grow each day! Join us as we onboard nonprofit, social service, and healthcare partners as the network continues to meet the needs of the community.

Unite Iowa will allow us to better serve families we encounter and help put necessary supports in place for those who we cannot serve directly. We are excited to be a part of this network and have better collaboration when working with those who have crossed the river in Pottawattamie County. Our hope is to see the continued expansion of partnering services working together as our communities thrive

The Unite Iowa network is bringing collaboration to our fingertips. We have joined the system in an effort to help families in our community access services even easier.

Helping people get well from addiction is the most important work we do. We were eager to join the Unite Us network to extend our services across the region. We often make referrals to outside entities – this is an important step to ensuring we’re meeting the needs of our members.

My heart is full. Girls Inc. joined Unite Us Iowa, an online portal designed to help people in need through referrals and collaborations. Our first referral was from the school district, helping a family with three children who lost everything in a fire. We wanted to thank your team for giving us this opportunity! The COVID-19 pandemic made it clear to our community just how valuable a shared client system would be. After researching different options, we felt that Unite Us had the perfect platform to provide our partners with a solution that we've been talking about for years. The Muscatine community is excited to start using the system to not only make the client referral process more efficient for providers but to serve those in need in a much better way. Network partners are now able to send and receive secure, electronic referrals in Arizona, Arkansas, Colorado, Illinois, Iowa, Kansas, Louisiana, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, North Dakota, Oklahoma, South Dakota, Texas, Wisconsin, and Wyoming through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.
